    The toes burn and tingle and sharp pains shoot into the legs. The bed sheets feel uncomfortable on your feet and you toss and turn, hoping to drift away into a pleasant slumber. But, the “fire-like” sensation in your toes keeps you awake. You switch positions, wiggle the toes, prop a pillow under the legs, but nothing seems to help. You can only lie in bed and watch the hours on the clock pass by.

    Background
    Burning, numbness, tingling, hot and cold sensations, shooting and electrical pain are common sensations felt in the feet in individuals with painful diabetic peripheral neuropathy (PDN). Neuropathy is an abnormality of the nervous system. Diabetic neuropathy is nerve damage caused by diabetes and is described as a loss of sensation that starts in the tips of the toes and gradually works its way up the legs. Diabetic neuropathy is sometimes referred to as a stocking glove neuropathy because it progresses as if one was pulling on a stocking. Diabetic neuropathy can affect both the hands and the feet.

    There are three types of nerves affected in diabetic peripheral neuropathy, sensory, motor and autonomic. Sensory nerves allow people to feel sensation, like pain, hot or cold or touch. Motor nerves control the muscles and allow movement. Autonomic nerves control bodily functions without our awareness or control. In the feet, an autonomic nervous response would be sweating. Neuropathy can affect all of these nerve group types, but sensory nerves are typically the first and most often affected.

    Statistics
    Almost twenty-one million Americans have diabetes. About sixty to seventy percent of diabetics have some type of nerve damage and about thirty percent of diabetics over forty years of age have diabetic peripheral neuropathy (CDC, 2005). Five percent of diabetics will experience painful diabetic neuropathy and the incidence increases with age. Over forty five percent of individuals who have had diabetes for over twenty five years will experience some symptoms of PDN.

    The Cause
    The exact cause of diabetic neuropathy is not clearly understood. Many theories exist, but the general school of thought is that high blood sugar causes chemical changes in the nerves and damages blood vessels carrying oxygen and nutrients to the nerves, impairing nerve function. A not so new theory, which is gaining in popularity is the idea that diabetics are susceptible to nerve compression. The susceptibility is thought to be due to the increased volume of the nerve from the abnormal glucose metabolism within the nerve. In layman’s terms this is as if the nerve is swelling and the surrounding tissues and ligaments are pressing on the nerves, resulting in a loss of function.

    Treatments
    Currently, there are no treatments to reverse diabetic neuropathy and there are no treatments that will eliminate the numbness. There are, however, many treatments to decrease the symptoms associated with PDN.

    Medications
    There was hope of reversing or significantly improving neuropathy with medications like aldose reductase inhibitors, myoinositol, protein kinase C inhibitors, C-peptide, vasodilators and nerve growth factors. Unfortunately, the research did not show consistent or effective results.

    The mainstay of medical treatment for diabetic peripheral neuropathy is to manage the symptoms with medications. For those individuals with painful diabetic neuropathy, prescription medications may be needed to manage the pain. Classic medications used for treatment include amitriptyline, desipramine and nortriptyline. These have been used to help decrease pain and to help with sleep. Fluoxetine, paroxetine, sertralene and citalopram tend to be better tolerated, but are also tend to be less effective at relieving pain.

    In September of 2004 the FDA approved duloxetine, known as Cymbalta. This was the first drug approved for specifically treating PDN. Gabapentin, also known as Neurontin, has been a successful treatment for painful diabetic neuropathy. Originally approved as an anti-seizure drug, it became a popular treatment in painful diabetic neuropathy. There was a controversy surrounding this medication when the manufacturer started marketing this medication to treat PDN, which is an off-label use. Many physicians still use this drug as a treatment. A newer medication has emerged called pregabalin, also known as Lyrica. Pregabalin was FDA approved to treat pain caused by nerve damage and many feel pregabalin is superior in effectiveness and has fewer side effects than neurontin. Tegretol and Dilantin, common seizure medications, can be used in more severe cases.

    Therapy
    Anodyne infrared therapy uses light energy to increase the circulation to the small vessels in the feet. Diodes are fit into flexible pads which can be applied directly to the skin on multiple areas of the foot. The theory is that light energy helps to increase blood flow which restores function to injured nerves. Initially released in 1994, Anodyne was met with mixed reviews, but has gained in popularity in more recent years. Some studies have shown great results with reduction in pain and relief of overall symptoms of neuropathy. Anodyne has also been used for treatment of diabetic ulcers, with some encouraging results.

    Surgical
    Nerve decompression has been increasing in popularity in recent years as a treatment for diabetic peripheral neuropathy. This surgical treatment was initially not met with much enthusiasm. Earlier studies showed poor surgical results and many surgeons were hesitant to perform elective surgery on diabetics. A new surgical approach has been introduced and surgical success rates have improved dramatically. The theory that diabetics are more susceptible to nerve compression, may warrant decompression of those nerves to give relief of the symptoms associated with diabetic neuropathy.

    Natural Alternatives
    There are numerous natural alternative treatment options for diabetic neuropathy. Many diabetics have had success with their use, but studies have not given consistent results showing their effectiveness. For painful diabetic neuropathy, red pepper powder can help decrease the pain experienced at night. Capsaicin is the active ingredient in chile peppers. When applied to the feet it acts as a counter-irritant and can help decrease neuropathic pain. Capsaicin can be purchased at your local drug store. Alpha lipoic acid is an effective anti-oxidant that has been shown in some studies to relieve pain associated with neuropathy. Gamma linolenic acid is an essential fatty acid, typically sold in the form of evening primrose oil. Most of the studies have shown modest results, but the possibilities are still encouraging. Side effects of long term use at higher doses may include inflammation, thrombosis (blood clots), or decreased immune system functioning. It is important to note that even if a product is labeled “natural” it does not necessarily mean that it is safe. Always talk with your physician before beginning any new supplements.

    Prevention
    As with all diabetic complications, prevention is the best treatment. Keeping the blood sugar levels within a normal range is the most important tool in treating and preventing peripheral neuropathy. Even with tight control, most diabetics will develop some level of neuropathy. Considering the severity of the complications associated with neuropathy (ulcers, infections and amputations), the associated pain in PDN, tight blood sugar control is of the utmost importance. Along with a healthy diet, exercising for 30 minutes each day will also help to increase circulation and stimulate the growth of new vessels, which may help slow the progression of neuropathy.

    Treating painful diabetic peripheral neuropathy is very difficult and many of the above mentioned therapies should be tried and combined. Don’t expect any “cures” and make sure you give each therapy a chance to work. It is recommended to talk with your physician or podiatrist to discuss these treatment options.

    ” Presume not that I am the thing I was; For God doth know, so shall the world perceive, that I have turned away from myself.” Shakespeare’s words from Henry IV, spark an interesting thought. How many times have I turned away from myself and felt a new me? So many scenes to replay in my mind. So many Me’s that were poor actors in my play, wanting to achieve fame and fortune without the integrity to turn away from myself. Masks of various emotions filled my playhouse of life, and touched the actors and the audience who watched them materialize, in many different ways. Each leaving with the presumption of the thing I was. I was so many things, as God doth know.

    All of the things I was, have gotten me to this place I call myself. Every event and interaction has made me grow into my own thoughts. The play has been for me. The actors and the audience have been learning tools to see myself not only externally, but to reclaim the other aspects of myself, that have been buried in the debris I collected, over the course of the play. My collection shined outwardly, but clouded my inner reality. After Acts 1, 2, and 3, I needed an intermission. I needed to change into my real self. The self I knew in practice, before the curtain of physical life opened.

    Really understanding the complexity of me, is a continuing performance. As Edmund Spencer pointed out in the 16th century; “For of the soul the body form doth take, for soul is form and doth the body make.” I can learn from his words and let my soul form the actor and the play. My thoughts script the performance and the playhouse. Each word and action are reflections of me, the soul, not the stand-in. The actors and the audience are attracted by my script and the message that I share with them. Soulful acting is not acting, it is being. Being is what I know and feel. My soul attracts a playhouse full of love. My ovation is my gratitude.
    The play never really ends unless I want it to, I can continue to perform my soul’s purpose in many ways. It is a choice I can make.

    Today’s performance starts with Act IV and it’s standing room only, every seat filled with love. My face glows in the radiance of remembering myself. My masks sit in a corner, gathering dust and cracking from neglect.

    In a recent conversation a colleague discussed doing motivational speaking. Then she said, “or maybe it’s inspirational.” That got me thinking about a common dilemma that managers and leaders face, “is it my role to motivate or to inspire?” To me, the two terms are very related but have a definite distinction.

    Motivation is something that comes from within. As a manager or leader, I don’t believe I can motivate you to do something, especially something that you aren’t interested in doing. Motivation is completely personal.

    What I CAN do is to create an environment that fosters self-motivation, based on precisely what motivates you - whether that’s money, responsibility, trust, empowerment, social meaning or something else.

    And that’s where inspiration comes into the picture, in creating that environment. Inspiration is an external factor. It’s the spark that lights up an individual’s motivators and sets them into action.

    One of the most inspirational leaders of our time was Martin Luther King. His words and actions connected with people’s internal motivators, specifically ethnic pride and social justice. He connected with people and inspired them with a vision of the future, and then showed them how they could be a part of making it a reality. Did he inspire everyone? No. There were many whose internal motivators didn’t connect or align with his vision. There was probably nothing he could do to have them take action.

    Think of some of the great energizing speakers or leaders that you’ve encountered. Did they inspire or motivate you?

    One of our jobs as a leader is to create an environment that inspires individuals to connect their own inner motivators to a collective vision. Here’s an example. A few years ago, I was coaching a banking executive, a regional sales leader. The collective vision of her group was to provide exceptional customer service while selling bank products and services to customers.

    One of the financial specialists in her group was not meeting his goals. To motivate him, the executive kept explaining to the specialist that he was leaving money on the table, incentive money, when he failed to sell these services.

    I asked the executive to tell me about the individual, and I quickly understood why this method wasn’t working. She told me that the individual spent a lot time volunteering in the community and enjoyed spending time with family and friends; he had been with the bank a long time and really loved to socialize with the individuals he worked with. I explained to the executive that it didn’t sound like money was a major incentive for this person and that it might be time to stop trying to motivate and start trying to inspire.

    To inspire him, she needed to tap into this man’s unique motivators, so I asked her to experiment by talking to the specialist about the future he was helping his customers create - the secure retirements, the college funds for their children and the first homes they were helping young couples purchase.

    In a month, the specialist was meeting and in some cases exceeding goals. When the executive stopped trying to motivate with what SHE thought would work and what motivated her, and began trying to inspire the specialist and tap into their own motivators, they found a common ground. She created a vision and environment where the specialist could be self-motivated and thrive.

    If you can not give yourself cash, at least give yourself credit. This is the fastest way to self confidence. Do you have any idea how much you have learned in your life time up until now. You have a wealth of knowledge at your finger tips. Just think for a moment what you have lived through up until now. I mean give me a break! You never made it this far without knowing something. You certainly do not need me to tell you what to do or how to do it.

    The problem with being human is we tend to forget how much we know. Nike says, “Do it!” Just go do what you have to do. You are building on all of your past knowledge. Doing and learning is the same word.

    The second problem with being human is, we pretend not to know. If I pretend not to know, I can take the easy way out. The truth be told, when you pretend not to know, that is the hard way out. Those who hesitate are lost! When you blink you sink. When you rest you rust and so on.

    A miracle is not coming. You are the miracle and you’re already here. You have to decide that. If it is going to be, it is up to me. Everyone that is breathing has circumstances. Never let your circumstances overwhelm you. Just keep kicking.

    When you stop kicking, they start dividing up all of your stuff. Everything you need is at your finger tips to what you must do. Talent does what it can, genius does what it must! Get in touch with the genius in you and shift your expectations into gear. Too busy? Too much trouble? Thinking about it too much? Too expensive? Too lazy?

    The opposite of lazy is willing. I am willing to listen, learn (do) and change how I think! What is it I need to know? Get a move on it! Do not wait to be motivated by punishment or reward. Greatness is being responsible. Being responsible is doing what is expected of you at all times. You are already GREAT! Give yourself credit for all you have done up until now. If you can give yourself cash, then by all means do so. Today you’re alive! Have no worry for tomorrow.

    Everything you already have is sufficient for today. Learning is the same word as doing.
    Fall in love with doing. The love of knowledge is still the root of all good. What you want the most wants you. Everyone is waiting for you to show up again, and again. Your excellent! Perfect is cold, excellent is warm, sometimes I am red hot!

    We often hear young people say, “I want to be free to do anything I want to do.” But, can one do anything he wants to do or to be? Obviously, one cannot wish to fly. We may invent a flying machine, but we ourselves cannot fly. Also, we cannot be free of problems. Being alive and human is to have problems. Even a professional bum has his or her own problems. We can try to deny that we have problems. But, in the end, these problems will just become bigger problems.

    Perhaps, there is another meaning to “be free to do anything I want to do.” For example, I want to be free to take illegal drugs; or, I want to kill a human being; or, I want to satisfy my sexual desires. But is one free to choose or not to choose to take illegal drugs? At first, one has the choice to take it. But, once one has been hooked to it, then really, one becomes dependent on it. One becomes addicted to it. There is no freedom in being attached to them.

    Out of anger, I want to kill a human being. If I act out of anger, am I free? Anger drives me to kill. If anger drives me to kill, then I am not free. Similarly, I may be able to choose my sex partner. But, it seems that I cannot free myself from my sexual hunger.

    What about a prisoner who is sentenced to life-imprisonment for the murder he had committed many years ago? Is he free? Or, can he be free? At first sight, we may think that he is not free for the simple fact that he is confined to the prison cell for the rest of his life. In other words, he does not possess physical freedom: he cannot physically liberate himself from the prison cell. Just like the addict, the killer, and the sex-starved individual, the prisoner is chained to his compulsion.

    Aside from being confined in the cell, the prisoner is confined to another kind of a cell. He desires to escape from the cell. He wishes for an escape everyday and every night. And the more he wishes, the more he becomes depressed and angry.

    How can he, and other like himself, be set free while being confined in his cell for the rest of his life? What can he be free from?

    There is another kind of freedom. It is a freedom from being attached to things in general. For example, I may not have any control over what the others think of me. Nevertheless, I have control over what I think the others may think of me. Let me give you another example: people grow old. That is natural. But, if I do not accept the fact that I will one day look like my grandfather, old with a skin like dried fish, then my desire to stay younger for a very long time, will imprison me. The desire to control things that are beyond my control will only give me more heartache. The desire to change the nature of things is like a boa constrictor: it squeezes me so tightly that I cannot move freely. These desires snatch my freedom away from me. Being free, therefore, is not about doing whatever one wants to do. Being free is about self-control, that is, to have control over one’s desires.

    If you have even the tiniest bit of web experience, you know that when it comes to obtaining exposure on the internet – search engines play a big role. You could have the most creative, informative website on the face of the internet, but if no one sees it – whats the point? This is why it is important to utilize optimization techniques. Using optimization techniques will help your website rise to the top of the search engines where it will be easily viewable to people searching for what you specialize in. Contrary to popular belief, optimization techniques are not hard to learn or implement. In fact there are two quick optimization techniques that you can utilize right now that will go along way in bringing traffic to your website.

    The first of the optimization techniques is – make sure you use keywords that result from a full keyword analysis. When doing a keyword analysis you want to initially have a large list of keywords you can choose from. In fact, you can go as far as creating duplicate pages on your website dedicated and optimized to misspellings. This is an easy way to pick up traffic without having to fight over keywords, and it will not hurt your overall appearance any because the only people that will see the page with the mistakes – are the ones who searched using the mistakes! Optimization techniques focused on spelling mistakes are not very common, so you can be assured almost free traffic. Many choose to use keyword tools in this process to make sure they are using the most profitable keywords. Keyword tools will not only give you variations of the keywords you want to use, but will also give you the most popular misspellings of these words and which words will bring more traffic than others.

    The second of the optimization techniques is – create a good TITLE right below the HEADER tag. This is the first thing search engines will look at to see if the page is relevant to a term that is being searched for. When implementing optimization techniques, if you have a good keyword or two in your title that competing pages don’t have, you will have a good chance of reaching the top of search engines. When going through optimization techniques, many website owners skip right over this and often waste potential by making very generic TITLE tags.

    For example: Welcome to VirtualOfficeAid.com – Home Page. This is very generic and not beneficial for search engine optimization. Using the words ‘welcome to’ is especially a waste of space, assume that your visitors will feel welcome without you having to say so – and use the space to tell search engines what your site is about! A better title for this website using optimization techniques would be: Administrative Aid, Website Design, Desktop Publishing, SEO. This gives the search engines and the reader information on EXACTLY what they will find on the website. Also when using optimization techniques in your title, try to place only 40-60 characters between the TITLE tags.
