Skin lumps are very common in dogs. There are a whole range of possible scenarios that can present as a lump on the skin, including, insect bites and stings, abscesses, hematomas, cysts, soft tissue swelling following trauma and benign or malignant tumors. Tumors are probably the most common of these, but two thirds of them are benign and therefore merely cosmetic. Here we look at the most common types of tumor affecting the skin and subdermis in dogs, and what, if any, treatment they require.

When presented with a skin tumor, a veterinarian has 2 issues to deal with.

1 Is it a type of tumor that requires surgical removal

2 Is further treatment required once it has been removed

Some skin tumors are so obviously benign that a vet will not even bother to biopsy it, let alone recommend removal. These are the small wart like tumors that are often attached by a narrow strip of skin. All skin tumors that do not fit this category though should be subjected to a fine needle aspiration (inserting a needle into the lump, sucking out some cells and characterising them under a microscope) or a surgical biopsy. This will determine whether or not complete removal is necessary, and define the prognosis.

A thorough clinical examination is important to assess both the tumor and the patient as a whole. A veterinarian will study the tumor with respect to its size, position and appearance, and examine the local and regional lymph nodes to look for any evidence of metastasis.

What is the difference between benign and malignant tumors?

Benign tumors are generally slow growing and may change little over the course of a year. They usually have a clear margin and cause the dog little trouble, but they can occasionally cause discomfort if they become ulcerated or start to bleed following self trauma.

Malignant tumors often grow rapidly over a few weeks. They tend to become ulcerated early in their course and they can infiltrate surrounding tissues, therefore seeming fixed and less moveable than benign tumors.

BENIGN SKIN TUMORS

Naevi and Skin Tags

These are not tumors, they are harmless, well marginated stable skin growths that are present at birth, but can grow in old age.

Warts and Papillomas

These are benign tumors that may grow spontaneously or may be triggered by a virus.

Sebaceous Cysts

These are common in dogs (around 7% of all skin tumors). They are not tumors, but accumulations of sebum within the gland due to a blocked duct, which can often be resolved by squeezing it.

Sebaceous Hyperplasia

This is proliferation of the sebaceous tissue mentioned above to form cauliflower like warts that may become traumatised, ulcerate and bleed. They are very common in old Cocker Spaniels and Poodles. Surgical excision or cryosurgery with liquid nitrogen is curative.

Basal Cell Tumor

More common in cats than dogs. Usually presents as a domed, well marginated hairless pink mass in the skin. They tend to be moveable over underlying structures and are slow growing. Surgical excision is curative.

Histiocytoma

Usually seen in juvenile dogs around 18months old, though can occur at any age. They grow rapidly over about 6 weeks to a maximum size of around 2cm, and are domed hairless lumps within the skin that can ulcerate. The key step for the veterinarian is to differentiate histiocytomas from mast cell tumors (see later), which can be easily done via a fine needle aspirate. Surgical excision or cryotherapy is curative for histiocytomas, and many will disappear of their own accord without treatment.

BENIGN SUBCUTANEOUS TUMORS

Lipoma

The lipoma is the most common subcutaneous tumor in dogs. It is a well defined, well circumscribed mass with a soft rubbery texture to it. They are slow growing and a very common occurrence in older overweight dogs. They can occur both on the trunk or on the limbs, and though they are completely benign, occasionally they can become so large that they are a nuisance to the dog and only then is surgical excision necessary. Before assuming a lump is a lipoma, it is good practice to have a veterinarian perform a fine needle aspirate on the lump as one or two other tumors can mimic a lipoma, such as certain presentations of mast cell tumors (see below) and hemangiopericytomas.

MALIGNANT SKIN TUMORS

Squamous Cell Carcinoma

This is a common malignant tumor in both cats and dogs. It usually occurs on relatively hairless non pigmented skin and can be triggered by long or repeated exposure to UV light. Surgical excision is usually curative if good margins are achieved, and those cases with incomplete margins benefit from post operative radiotherapy to kill the remaining cancer cells. Some squamous cell carcinomas are very difficult to remove though, such as a nasty infiltrative form which occurs on the nasal planum. Chemotherapy has been used for this tumor type with mixed results.

Melanomas

Relatively common in dogs. The majority are benign and surgical excision is curative. However, those that arise on the nail bed, scrotum or mucocutaneous junction (e.g. on the lips) are usually malignant. Surgical excision is mandatory.

Mast Cell Tumors

These are amongst the most common of skin tumors in dogs. They have a wide range of appearance and behavior, making them sometimes challenging to diagnose and treat. About 10 per cent of dogs suffer them at multiple sites.

Mast cells occur naturally in the skin, performing an important function in releasing histamine and heparin in response to various allergic stimuli. Like any cell line, mutations can occur where the cells start to divide uncontrollably, forming a tumor. If a mast cell tumor is squeezed it will therefore release histamine and cause a wheal reaction on the skin. Histamine can also cause vomiting so affected dogs may present with upset stomachs.

If a mast cell tumor is confirmed on a fine needle aspirate, it should be surgically excised as soon as conveniently possible. In cases of well marginated tumors that are caught fairly early, this is usually curative. Margins of 2 to 3cm are advised. However, higher grade tumors tend to be aggressively invasive and complete excision is not always feasible. In these cases, partial excision together with radiotherapy or chemotherapy is the treatment of choice.

Dogs that have had mast cell tumors tend to be predisposed to getting them again in the future, even if they are surgically excised promptly. Therefore, owners of such dogs should meticulously examine their dogs skin at periodic intervals to search for any suspicious lumps.

When we are first starting out we have a tendency to overspend and we quickly begin to run through our limited funds in record time and create an enormous amount of debt and even more so if it’s not our money. What would you spend your money on if you knew it was your last one hundred dollars? That you were not going to get more money for a long time? You would focus only your most pressing needs. You would be forced to assess each area to find out if you are getting the results that would justify spending your last bit of funds.

You would try to find or create ways to promote your business that didn’t require a lot of expense. Labor intensive rather than spending any more money. What could you do?

You could barter for ad space; you could carry the flyers yourself instead of paying a kid to do it. You could cold call rather than spend it on advertising, you could place cheap ads in e-zines or on-line newsletters, you could collect e-mail and send out a free newsletter every month, you could target your advertising to your local area, you could team up with other local businesses for referral business, you could cross promote, you could make presentations for the chamber of commerce for free and see if they will pick up the cost of the event, you could call the local library and see if you can make a presentation or speak on a topic that could be noteworthy or of interest to the general public, you could write articles or a column in your local paper with consumer tips, or post articles on-line about your specialty.

Of course we live in the real world of everyday expenses and you have to spend money when you are in business on delivery charges, utilities, phone services, monthly services, classes or seminars, emergencies that unexpectedly happen, business trips, and car service to the airport sometimes.

But by budgeting those things into your calculations and keeping money on hand in case of an emergency it will conserve your funds and force you to think more creatively. Now when you are thinking of taking a risk with that last $100 you will have a pretty good idea if it will fly. What you have to do is picture where you want to go and what you want to achieve. With the last of your dollars will you achieve a big windfall or pull in an expanded income?

Even if you have thousands of dollars coming in per month it is a good idea to assess each area of your business to see if you can improve the processes to make it more efficient, that you can save money by implementing a new feature, or that you are making the kind of money you envisioned, with the new improvements.

In the end when you are making money remember to replace your emergency funds, pay down your debt and continue to improve your services and also keeping your costs budgeted, and keeping a check on your credit card spending. Paying high credit cards is not where you want to spend your last one hundred dollars.

When we are making a little more money is also the time that we are tempted to overspend. We justify this by saying I have a lot of money coming in now. Be warned that everything can be fleeting if you don’t set up a budget, manage your expenses, and keep a good eye on your results.

American history buffs know the sad but true story of William Henry Harrison’s short tenure as President. Mr. Harrison chose not to wear a hat to his inauguration; he caught pneumonia and died within the month. Twenty or so years ago - sometime during the Reagan administration - I created a hangtag for our stores that read “Hats Are Healthy”. By it, I meant not against the cold but rather against the harmful effects of the sun. I quoted two agencies (see the tag at the Great Hats For the Great Outdoors page at our site). I was ambivalent about these tags because on the one hand, I wanted to call people’s attention to the need to put on sunscreen and wear hats (and other protective clothing), but on the other, I felt guilty profiting from fear. Furthermore, our hat business had been successful because our stores were fun and although the “hats are healthy” message was valid, I didn’t want to overemphasize it. (An atmosphere of a serious clinic was an unproven model for a successful hat shop.) In any case, I figured that in short order most people would get the message anyway and be wearing hats, along with sunscreen, and therefore, calling attention to headwear as protective clothing would soon be as unnecessary as calling attention to the merits of sunscreen.

Two decades have passed since that hangtag. I go out and about in life as we know it and the fact of the matter is that most people are still not getting the message - not wearing hats. And I live in San Diego - where there’s lots of sun. So here’s the current data: The National Cancer Institute estimates that in 2007 there will be more than 1,000,000 new cases of skin (non-melanoma) cancer in the United States and less than 2,000 deaths. NCI also estimates 59,940 new cases of melanoma (a form of skin cancer that begins in melanocytyes - the cells that make the pigment melanin) in 2007. Of those, NCI estimates 8,110 deaths. When asked about the effectiveness of sunscreens in preventing cancer, Dr. Kenneth Kraemer, a dermatologist researching skin cancer for NCI’s Center for Cancer Research replied, “Sun protection is more than sunscreens. Sun protection includes avoiding the sun, using clothing to protect yourself, getting shade under a tree if you are going outside, and wearing hats”.

As we enter spring 2007, I’ve decided it may be important to amplify the alarm.

Tip 1: Employers will assume that you have done a good amount of research on their company. When they ask you what you know about them, you’ll get off to a good start by being able to discuss several pertinent points about the Company. To do this you must be sure your research includes:

* What the company’s mission is
* Sales and revenue (if public)
* What their growth rate has been
* Major products and services
* Who their competitors are
* Key management team
* Latest news on the company
* Clients, suppliers
* Company structure & culture

Employers want to know that you are interested in working for them and that you have the curiosity and energy to plan ahead for your meetings. This research technique will also help you come up with smart questions to ask your interviewer when they offer to answer any questions you may have. P.S. Always have questions!

Tip 2: Employers expect you to be able to discuss your strengths, give examples of your strengths in action and tell how your strengths will benefit them. In addition to your academic performance and experience (internships, etc., if applicable) a few other areas assessed during an interview are:

* Team work - how will you work and interact with others
* Self knowledge - a realistic assessment of who you are
* Personality - how will you fit into their culture
* Image - how you present yourself as well as your overall appearance
* Work ethic - reliability, integrity
* Confidence - ability to openly discuss goals and accomplishments

Tip 3: Employers expect you to tell them “What’s In It For Them” if they hire you. Gone are the days when you ask the employer why they think you should work for them. Be ready with 3 top reasons you are a good choice for the employer.

Tip 4: Employers will expect you to be able to provide them with some self-disclosure so they can get to know you better. You should be prepared to answer questions around:

* Weaknesses (and never use something lame like - “I just tend to work too hard” that’s a real eye roller)
* Your problem solving abilities
* Extra curricular activities
* Community involvement and/or volunteer services
* Why you chose your career field and information about your long term career goals

Remember, employers want top talent and you must be able to articulate your value and the benefit you will bring to an organization. Relationships and dating have always gone together - hand in hand no matter what the generation. You meet, you date, you fall in love, and if you’re lucky or so inclined, you get married. What’s missing from your LMS RFP?

If you have been involved in sourcing or researching Learning Management System vendors in the past few years, you may have noticed that your functionality spreadsheets and RFI responses all look the same. If you think about it, this makes sense. Now that the number of vendors in this space has gotten closer to reaching earthly proportions, you would expect all vendors to meet the common requirements. Your task at this point, to narrow the field or identify a clear leader becomes quite difficult. It’s your responsibility to not only spend your company’s money wisely but also to put the right software behind the objectives.

Here are some tough questions that are not asked often enough of vendors. Some of them may make your salesperson squirm, but in the end will help you be more confident in your decision and avoid all-to-common pitfalls. This list should not be used verbatim nor is it intended to be complete. Pick the questions that are important to you and add them as you see fit. Most of all, keep a sharp eye out for candid responses. The vendors who are forthcoming with this information are one’s that deserve serious consideration.

What Post Implementation Costs Will We Potentially Incur?

Some vendors report as much as 65% of annual revenue from professional services. Why should this alarm you? Because you don’t want to more than double your initial investment to get the system to work the way you want it to. Find out what aspects of the system you have control over and what you will have to rely on (or pay) the vendor to do. Will you pay a fee to upload content? What will you have to pay if you choose to create a sub-portal to serve a unique user base? You’re responsible for your project budget, don’t let hidden costs derail your efforts to serve your organization.

Describe your 3rd Party Partner Relationships

There will always be a valid debate between best of breed vs. software suites. I could provide a compelling argument for either. Instead I will offer this insight: If your chosen platform includes 3rd party components, what does that mean to your support? Do you have a direct line of communication with those vendors? Do the 3rd party vendor’s product plans mesh with your LMS vendor’s? Is there potential for future competition between the parties? Will new releases always be in-synch to avoid any software conflicts? OK, so I just made a compelling argument for the latter. Regardless, this is a must-know.

How many clients are using the latest release of this exact platform? Provide references using the exact version of the software you plan to sell me.

With all of the mergers and acquisitions that have taken place in the LMS space, it’s not uncommon for a company to support several applications. Knowing if the vendor has a couple of dozen clients on each, is in the process of moving all clients to a single platform ($$) or phasing out products will give you a good read on what you may be in for during the term of your contract. I would also want to know that the household name references I was provided actually use the exact software I intend to purchase.

What impact will customizations have on future upgrades?

6 months after deployment, you finally have the application behaving exactly the way you want it. You and your team have put tremendous effort into building out your system. Your HRIS integration works flawlessly. Now, what happens when an upgrade is pushed out in the middle of the night? Will your system revert to its out-of-the-box existence? Will you be put into a queue until the vendor can provide you the manpower to make the migration? Worst of all, will you be forced to live with the current release while new clients get all of the new bells and whistles?

Describe your Support & Development Services (including location and escalation process).

Outsourcing is commonplace and I do not feel it should be weighted any differently than the domestic equivalent. I would like to know however if the support team and development team are in different hemispheres. I would also like to know specifically what support resources are available to me and how much client feedback goes into future product development.

How will integrations be supported on future releases?

At some point you are going to upgrade or change your existing enterprise applications. You will want to be certain you know what impact that will have on both one-way and bi-directional integrations with your LMS and what support will be made available to you to manage this process. Forward thinking vendors have chosen integration technologies which will require very little (if any) reworking to keep the data flowing.

Provide product release notes for the previous 12 months, next 12 months.

First, I want to see what you have done with the product over the past year. Are most of the changes enhancements or repairs? I also want to be wary of any large gaps in development. Stagnant software could be a sign of bigger concerns. I also want to know where the product is going. I’m going to feel a little silly buying a shiny new competency library, only to see it included in the next release of my LMS. Also be wary of any end-of-life releases in the vendors recent past. This scenario has caused countless headaches for otherwise faithful clients who are now forced to adopt the new (and sometimes unproven) technology, or stay with an obsolete and unsupported version.

Describe any major changes within your organization, and how this has impacted your clients?

If hundreds of clients were forced to upgrade to a different platform because of a merger or acquisition, well, I just want to know. I liken this to your car dealer calling and telling you that your new car was going to blow up in 6 months, but for a few thousand bucks you could come down and get on the waiting list for a new model.

How does your licensing model support infrequent users?

Many companies have large populations of learners who will be accessing the system infrequently (annual certification, compliance requirements, etc.). Your chosen vendor should offer a licensing model which takes into account that your bandwidth requirements and use of the system may fluctuate. There is no good reason to pay an annual fee for users who will access the system once a year. You may also consider asking about ways in which the “deactivation” of users can be automated to manage your licenses.

The Learning & Talent Management purchases that you make will have sizable and long lasting impact on your organization. We are fortunate to have learned from the first generation of LMS and eLearning licensing when we all became highly aware of our overzealous buying habits. I hope that we will continue to learn from our mistakes and share these ideas with our peers. All told, the Enterprise Software arena is high-stakes for all involved. While these types of questions appear only in a small portion of the RFP/RFI’s that come across my desk, I feel that they are imperative to making a sound decision. This is certainly a short list and I encourage you to share additional pearls of wisdom with your peers in the learning community.
