All surgical procedures carry risks and, while the laparoscopic adjustable gastric band in general carries fewer risks than many other forms of weight loss surgery, there are nevertheless risks which you must be aware of before deciding to undergo surgery. Here we look at some of the general risks but it should be emphasized that each patient will also encounter risks specific to their own case and these will be explained to you by your surgeon.

Gastric band risks can be broadly divided into two categories - the risks associated with the surgery itself and the risk of post-operative complications.

The first and most important risk is that of death which can occur either during or following surgery. In the case of gastric band surgery this risk is extremely low and indeed, during US clinical trials on laparoscopic gastric banding, no deaths occurred. Major surgery in any form however will always carry this risk and this should be borne in mind.

The second major risk during surgery is that of gastric perforation. A tear in the stomach wall can occur during surgery (and occasionally following and normally as a direct result of surgery) which will invariably require further surgery to correct the problem. Once again the risk is low in the case of gastric band surgery and can be expected to be seen in about 1 percent of patients.

Other surgical risks are those associated with surgery in general and will include such things as the risk posed by age, excessive weight and the presence of pre-existing conditions and disease. There are also risks associated with medication used during the procedure (including anesthesia) and the method used during surgery. It should be noted that while we talk about laparoscopic gastric band surgery as if it were one specific operation there are in fact many different ways in which the procedure can be conducted depending upon the medical facility in which the operation is carried out and the surgeon performing the procedure.

The majority of complications following surgery are not considered to be serious but there are some that may require hospitalization and, occasionally, further surgery. The most common post operative complications are nausea and vomiting, which is seen in about half of all patients, regurgitation, which is seen in about a third of all patients, gastric band slippage and associated problems, which is seen in about a quarter of all patients and stoma obstruction, which is seen in about 1 patient in 7. Other commonly occurring problems seen in about 1 patient in 10 include poor esophageal function, constipation, diarrhea and difficulty swallowing. Slightly less than 1 patient in 10 is likely to need a second surgery to adjust the gastric band or to correct other problems arising out of the initial surgery and a slightly smaller number are likely to require further surgery to correct post operative problems with the banding system’s access port.

In addition to these common problems there is also a very long list of minor complications which will be encountered by a small number (less than 1 in 100) of patients. These include such things as gastritis, hiatal hernia, dyspepsia, flatulence, dehydration and fever.

As with all forms of surgery, gastric band surgery is not without its risks and these must be fully understood and weighed against the benefits of surgery.

Rash is the growth of red bumps on the skin and is often caused by irritation. Most rashes cause the skin to become swollen, itchy and develop scales. Though many rashes are not contagious, they can be affected by many external factors such as dust, diet, animal hair, clothes, and many other similar things.

Rashes can also develop as a result of the skin getting too dry, or certain skin allergies. Infections and fungal growth can also trigger many rashes.

The most common cause of rashes is the skin drying out. People that suffer from dry skin are more prone to develop some form of rash.

People suffering from any form of rash should not take hot baths or showers. Lukewarm baths are ideal. Lukewarm baths cleanse and also prevent the skin drying out excessively. Ideally soap should be avoided, however if required, a mild soap could be used. Try and find a soap specifically designed for sensitive skin.

After getting out of the bath, your skin is at its most vulnerable, to avoid drying of the skin, a moisturizer should be applied. This is one of the easiest things to do!

Certain allergic reactions can be another cause of rashes, commonly people are allergic to animal fur, fabrics, nickel or several other heavy metals. Normally only the area that comes into contact with the substance that you’re allergic to is affected. To resolve this problem you should try to work out what you’re allergic to and stay away from it.

Rashes can also be caused by fungal growth on the skin. Fungal infections are pretty rare in modern society however it’s nothing to do with poor hygiene. In fact people with excellent hygiene may also suffer from fungal infections. Antifungal cream should be applied to the rash at the affected areas. These antifungal creams are easy to obtain and are available at any local drugstore.

So there you have it, although many rashes are not curable it is possible to relieve the symptoms of them. Remember to sit on your hands! Whatever you do don’t scratch! Scratching only worsens the condition.

In my earlier years of playing guitar, I could never even begin to comprehend how guitar players could jam with other musicians and improvise, when it came their turn to take a break. I would listen and watch in amazement as a guitar player would (effortlessly, it seemed) play lick after lick of great sounding lead guitar. Then I read about “the blues scale.” This simple scale, the blues scale, which I learned in a day, and mastered in a week, opened up an entirely new world to me. By knowing this collection of notes which would go with a given chord, I was able to do amazing feats of improvisation in a few days!

This collection of notes can be played in any key and position on the guitar, but to begin, let’s try it in the key of A, in the fifth position (with your left hand moved up to where you are covering the 5th through the 8th frets).

Here is the Blues Scale in A…

On the 6th string (the low E string), play a note using the first finger on the behind the 5th fret, and then a note using the fourth finger (”pinky”) behind the 8th fret.

Then, on the 5th string (the A string), play a note using the first finger behind the 5th fret, then a note using the second finger behind the 6th fret, then a note using the third finger behind the 7th fret.

Then, on the 4th string (the D string), play a note using the first finger behind the 5th fret, then a note using the third finger behind the 7th fret.

Then, on the 3rd string (the G string), play a note using the first finger behind the 5th fret, then a note using the third finger behind the 7th fret, then a note using the fourth finger (pinky) behind the 8th fret.

Then, on the 2nd string (the B string), play a note using the first finger play a note using the first finger behind the 5th fret, then a note using the fourth finger behind the 8th fret.

Then, on the 1st string (the high E string), repeat the sequence you played on the 2nd string-that is play a note using the first finger play a note using the first finger behind the 5th fret, then a note using the fourth finger behind the 8th fret.

Play this using alternating picking strokes (downstrokes and upstrokes), and go up the scale (from the 6th string to the 1st string) and then down the scale (from the 1st string to the 6th string). Then, fool around with it-play different sequences, but use only the notes on this scale. It may open up a whole new world to you too!

Are Ethics Important For Professional Accountants?

Ethics in professional accountancy are of utmost importance. Now as the business and financial world is adopting international accounting and auditing standards, it is becoming all the more necessary to adhere to certain Code of Ethics prescribed by international and national accountancy bodies. Before arguing in favour of the topic, let’s have a look at some basic concepts:

Profession

A profession is an occupation that requires extensive training and the study and mastery of specialized knowledge, and usually has a professional association, ethical code and process of certification or licensing; for example engineering, medicine, social work, teaching, law, finance, the military, nursing and Accountancy etc. Classically there were only three professions: military, medicine and law. Each of these professions holds to a specific code of ethics and members are almost universally required to swear some form of oath to uphold those ethics, therefore ‘professing’ to a higher standard of accountability. Each of these professions also provides and requires extensive training in the meaning, value and importance of its particular oath in practice of that profession.

Accountant

Practitioner of Accountancy is known as Accountant. Qualified Accountant, Accountant, Professional Accountant or Accountancy Practitioner is a legally certified accountancy and financial expert. Accountants not only work in public practice but many of them are working within private corporations, in financial industry and in various government bodies. Accountancy (profession) or accounting (methodology) is the measurement, disclosure or provision of assurance about financial information that helps managers, investors, tax authorities, lenders and other stakeholders and decision makers to make resource allocation and policy making decisions.

Like many other professions there are many professional bodies for accountants throughout the world. Some of them are legally recognized in their jurisdictions such as British qualified accountants including Chartered Certified Accountant (ACCA or FCCA), Chartered Accountant (CA, ACA or FCA), Canadian qualified accountants such as Chartered Accountant and Certified General Accountants (CA or CGA) and American qualified Accountants such as Certified Public Accountants (CPA) etc. Some other statutory and non-statutory accountancy qualifications are Certified Management Accountant (CMA), Associated Cost and Management Accountant (ACMA), Certified Financial Analyst (CFA) and Certified Fraud Examiner (CFE) etc.

In Pakistan, the Institute of Chartered Accountants of Pakistan is the sole professional and accountancy body with the right to award the Chartered Accountant designation. ICAP is the member of IFAC (International Federation of Accountants, IASB (International Accounting Standards Board), Confederation of Asian & Pacific Accountants (CAPA) and South Asian Federation of Accountants (SAFA). The members of ICAP have reached to 4,089 as of March 1, 2007 data.

Role of Professional Accountants:

Accountants are independent business advisors. Accountants can offer an extensive range of services. Accountants can be registered auditors, can set up client’s accounting systems, can be an advisor on tax planning, or a detector of frauds and embezzlements, can do budgeting and financial statement analysis, advise clients on financing decisions, provide specialist knowledge and can help maintaining an ethical environment.

After discussing the basic concepts and role of professional accountants we are in a better position to ponder on what professional ethics is and why it is important in the field of accountancy.

Definition of Ethics

The word ‘Ethics’ is derived from the Ancient Greek word ethikos; means customs and habits. A major branch of philosophy which is the study of values and customs of a person or group and covers the analysis and employment of concepts such as right and wrong, good and evil and do’s and don’ts.

Code of Ethics:

In the context of a code adopted by a profession or by a governmental organization to regulate that profession, an ethical code may be styled as a code of professional responsibility, which may dispense with difficult issues of what behaviour is ‘ethical’. A code of ethics is often a formal statement of the organization’s values on certain ethical and social issues relating to the profession and practice of the professional knowledge. This also includes the principles and procedures for specific ethical situations.

Ethics in Professional Accountancy:

The general ethical standards of society apply to people in professions such as medicine, law, nursing and accountancy etc just as much as to anyone else. However society places even higher expectations on professionals. People need to have confidence in the quality of the complex services provided by professionals

Ethics in accountancy profession are invaluable to accounting professionals and to those who rely on their services. Stakeholders including clients, credit grantors, governments, taxation authorities, employees, investors, the business and financial community etc perceive them as highly competent, reliable, objective and neutral people. Professional accountants therefore, must not only be well qualified but also possess a high degree of professional integrity. Because of these high expectations, professionals have adopted codes of ethics; also known as codes of professional conduct. These ethical codes call for their members to maintain a level of self-discipline that goes beyond the requirements of laws and regulations. Each of the major professional association for accountants has a code of ethics.

As mentioned earlier, professional accountants can be of two types. One who work in firms or independently run those firms that provide accounting, auditing and other advisory services to clients; these are called public practitioners. Others are those who are employees of organizations and may serve as internal auditors, management accountants, financial managers and financial analysts. Regardless of the role of accountants, they are adhered to code of ethics which are applied to their professional conduct although there are some special provisions for those in public practice [Reference: Code of Ethics for Professional Accountants-International Federation of Accountants (IFAC)].

International Federation of Accountants-IFAC:

The International Federation of Accountants (IFAC) is a federation of all accountancy bodies throughout the world. All the major international and national associations like ACCA, AICPA, ICMA, ICAP, IASB etc are all its member organizations. The mission of IFAC, as set out in its constitution, is “the worldwide development and enhancement of an accountancy profession with harmonized standards, able to provide services of consistently high quality in the public interest” [Ref: Code of Ethics for Professional Accountants-IFAC]. In pursuing this mission, the IFAC Board has established the IFAC Ethics Committee to develop and issue, under its own authority, high quality ethical standards and other pronouncements for professional accountants for use around the world. The Code of Ethics establishes ethical requirements for professional accountants. A member body or firm may not apply less stringent standards than those stated in this Code.

The objective of setting this code of conduct is to harmonize these standards and practices on a global perspective. Public can only trust these highly professionals when it is made mandatory to observe and follow strict regulations and codes throughout the world. A professional accountant is required to comply with the following fundamental principles mentioned in this Code of Ethics:[Ref: Section 100.4 Code of Ethics for Professional Accountants]

·-Integrity: A professional accountant should be honest and straightforward in all professional and business relationship.
·-Objectivity: A professional accountant should not allow bias, conflict of interest or undue influence of others to override professional or business judgments.
·-Professional Competence & Due Care: A professional accountant has a continuing duty to maintain professional knowledge and skills at the level required to ensure that a client or employer receives competent professional service. A professional accountant should act diligently and in accordance with applicable technical and professional standards when providing professional services.
·-Confidentiality: A professional accountant should respect the confidentiality of information acquired as a result of professional and business relationships and should not disclose nay such information to third parties without proper and specific authority unless there is a legal or professional right or duty to disclose. This information should not be used for personal advantage by professional accountant.
·-Professional Behaviour: A professional accountant should comply with relevant laws and regulations and should avoid any action that discredits the profession.

Code of Ethics defined in ‘Members Handbook’ for members of ICAP Pakistan is in conformity with:
·-IFAC Code of Ethics and International Auditing Standards
·-International Accounting Standards
·-The Institute of Chartered Accountants of Pakistan - ICAP
·-Relevant legislation

This Code of Ethics has discussed in detail the role of Chartered Accountants in given situations. For example there are clear directives on prohibition of acceptance of gifts, long association with clients, advertising of firm’s name exceeding prescribed limits, holding client’s monies for no sound reason, disclosure of client’s records (except ones that are allowed), acceptance of fees offered by client which is less than that prevailing in market etc.

After discussing in detail the importance of ethics in accounting profession, we are to conclude the topic with this final note that accountancy as a profession is acceptable and relied upon only when ability to exercise professional judgment based on a foundation of ethics; broad but deep technical excellence and strategic awareness are exercised by a professional accountant. Only then general public can trust the integrity of this profession.

Interlude

KP

KP, or call it Kitchen Police, Kitchen Duty, or whatever, but back in my day, ever soldier did it. I was woken up this one morning of my seventh week in training, it was a Sunday, and someone wanted to go to church, so guess who they picked for kitchen duty, me. I wasn’t supposed to have it, I had had it three times before, and was suppose to have been done with it. But the Army never works that way, they just keep putting straws on the camels back until he drops, or says something to stop it, and I was not everyone’s favorite soldier, so I just accepted it, I was close to going on to the next stage, advance training in Alabama so I figured another day on KP would not hurt. Yet at the time I didn’t know my next duty station. I didn’t even know if they were going to pass me, I mean, they could have fixed it for me to stay around a while if they hated me so much, you know, torment me with another eight weeks of this boy scout training as I had felt it was. They had done it I heard, but they would not do it to me. Although I’m getting ahead of myself, it is of no consequence to the story here and beyond.

“Soldier, get up, you got KP!” said the young sergeant, my drill sergeant, at 4:00 AM, with a smirk on his face. He was a vulture, “I already had it three times before!” I said.

“You got ten minutes…no more!” he added to his unsightly face. The Buck Sergeant stood outside, waited to see if I was coming, and I was, I rushed to and fro…and was on my way in ten minutes flat.

It was as if by me staying in the platoon touched off a high explosive inside the sergeant’s head, I think he would have liked me to have gone AWOL, run to Canada for his amusement. As I walked outside, onto the dirt road in front of the barracks, and then on down the dirt road, and across the black asphalt road-that went the opposite way, to the Mess Hall, he looked a bit gloomy, I was turning out to be a soldier indeed, and he wasn’t sure if he liked that.

It was a long day, or would be. First came the dishes, then the pots and pans, and then the potatoes, yes, I hated doing the potatoes, not because it was hard, nothing in the Army is that hard, it was boring, and they had an automatic potato peeler right behind me, staring at my back side, as I sat on the steps in back of the mess hall, peeling potatoes the old fashion way, with a knife, slowly, and a big pot for the skins of the potatoes and one for the potatoes. I think it was based on not wanting us to have something to do, rather than nothing to do and the automatic peeler would only do the job quicker and allow us to have free time. Oh well, it was all part of the show I told myself. And it gave me time to think of many things.

(I thought about Maria Garcia, a young woman I was seeing and had met while on Christmas leave, back in St. Paul (the past December). She had a kid, and we’d drink a lot together, and she always seemed to be having family, friends, people in general over to her house, a Mexican thing I think, or Spanish thing, more the company the better; where as for me being the gringo, I was not used to this, and had I suppose less of a family life in that I didn’t have so many people around, more of a loner. But it was nice meeting everyone. She was cute, short, black thick hair, a nice shape on her, and somewhat of a decent lover. And I never told her I was in the Army, and on my last day of leave, I simply left, that was it, I got up one morning, had my orders to go, and left, never even made a phone call, had I, I would not have known what to say anyhow. I would see her some two years later; she’d spot me in St. Paul, in a grocery story, and ask, “Whatever happened to you?” She wasn’t even mad, just concerned. I replied: “I’m really sorry, I was on my way to Vietnam, to war, and I thought, had I told you, it would just get in the way.” Well there was some truth to that, I had went from Fort Bragg, to advance training in Alabama, and onto West Germany, before I went to Vietnam, I kind of let all that stuff out of the picture, deleted it you could say, and just added Vietnam, and war.

“Oh my gosh,” she said, with a serious look.

“How are you doing now?” I asked. And she assured me she was doing fine. Evidently, living with someone, and thus, we parted good friends.

On my three hundred and forty-forth potato, I got thinking about Sergeant Wolf, a black sergeant, drill sergeant that is. How he’d smoke, solemnly smoke them cigarettes, right to its end. He was there among the other Drill Sergeants often, talking, he was from ‘C’ platoon, I think he liked me, because I made him look good, and our sergeants bad; they always had bets, betting on this and that: saying there platoon was better, and I think my drill sergeants lost many bets. He had a fleshless neck, all most none at all, and a head of an absurd largeness; a stooping body like an ape, and hands that almost touching the ground when he walked. He was the Judo and Karate instructor; I could have taught the men better, but for what time we had, it was good enough. I think at times his prerogative was to out show me, but whatever he showed, or demonstrated, I could do better, he had a horrible agility, dull small eyes, clean-shaven. He darted here and there it seemed, like a spider, stupidly I often found myself looking at him. I wouldn’t miss him, I told myself.

Yes indeed many thoughts were going through my mind this day, this twelve hour day: I remembered the three Generals, the second or third day I had been in boot camp, Smiley, I and Bruce were sitting down in the clothing supply area waiting to get sized up for our dress greens, and here comes three generals, I didn’t really know a general from a captain, but one had three stars on his shoulders. “How they treating you soldier?” he asked me, I didn’t get up, and simply said, “So, so, I guess,” he smiled, and said something else, and I never saluted him, nor stood at attention, that was a peeve with my young drill sergeant, but he got over it, after warning me, should it happen again, I’d be severely reprimanded; the General saw the sergeant was upset, and told him in so many wards: give him a break.

The other thing that came to mind in my daydreaming was the old sergeants appearance, my drill sergeant, when I say old, I do not really mean, old, old, but for a drill sergeant, old: he had a square jaw, like me, but was a few inches taller, not much, a rough looking face, as if he had been around a bit, small eyes, half closed all the time, or seemingly so. At times he was vigorous and at times a cold pathetic look gravitated all over his face to his forehead. He was what many called, a Red Neck, perhaps thirty-seven years old, but he was a vulture nonetheless.)

Army Life

I felt at times I was the side focus of the group of drill sergeants, they had beat the hell out of one of the soldiers for not adjusting and getting smart with them, which I really never did, I mean I never disrespected them verbally, I was simply not afraid of them, and they knew it. Moreover I was guarded I suppose, waiting for them to do it to me, or try. And they knew I was waiting, and I think my eyes warned them, be careful, you are treading on unknown ground, and somebody besides me will get hurt also. What I took to be men of honor, among our leaders, disappointment me somewhat, most were fine, but some were not. They had a job to do I know, and this is of course how I was feeling at the time: everyone with gaunt and hard eyes, with gloomy jobs, and often drunk before lights went out for us. The older drill sergeant, my drill sergeant couldn’t talk for two weeks, laryngitis (inflammation of the larynx). Not sure why I thought this was funny, but he couldn’t holler like he’d have liked to.

At the end of the day, I had a few aches and some numbness, my muscles danced, and my nerves wiggled. Smiley came by once, said: “See yaw at the beer hall tonight…!” And Bruce and Allen would be with him. Both good old southern boys, as they called themselves. Allen was a large figure of a man, glasses and smart. I nodded my head ‘yes’ and kept on peeling those potatoes, and cutting them up.

The cold turbulent winds of winter have ceased. For months it settled on the earth, the greenness of life faded, trees in full bloom now barren; and we lock ourselves indoors or in heavy coats craving protection from the elements. Of such too is the winter of life - cold, lonesome, desperate, tragic. A time of shaken confidence, unanswered prayers and blurred vision. Spring is now here. A welcome respite from the bleak landscape that winter brought. The snow is melting, wild daisies are blooming and the birds are returning from their retreat in warmer climes. Spring is a season of life and bounteous beauty. It is the season of opportunity and activity. It is not time to linger and ponder the success or failure of past seasons but rather a time to launch with seeds of commitment, knowledge and persistence.

A time to choose

When the dark clouds of winter have lifted and the warmth of spring has arrived, we are left with one of the most powerful gifts given to us by the creator - the power to choose. We can choose to wallow in self pity because of the severity of the winter or choose to be encouraged by the new opportunities available to us. We can chose to be put down by the harsh opinions of others or instead chose to acknowledge our greatness and our innate ability to surmount any challenge that life may throw at us. As much as we despise them, we can choose the familiarity of our current circumstances or instead choose to be courageous and risk the chance of failure in pursuit of a better tomorrow. It is only through honest hard work that one is able to reap the rewards from the opportunities that life presents us. Half-hearted efforts lead to a mediocre life while taking action translates into a full storehouse. It is time to enroll in a class, switch careers, start a business, and adopt healthier habits in order to live the life we dream of.

A time for faith

While spring only guarantees that opportunities to recover from the brutal winters exists for us and that through hard work we can convert those opportunities into results, life provides no assurances that our concerted efforts will reap bountiful results. Life’s snowstorms could easily negate the massive effort we made to turn the opportunities into results. Conversely, failure to make an effort will most definitely guarantee no results come harvest time. As Jim Rhon says, “much of the effort and opportunity of springtime rests in the depth and degree of our faith.” Faith is the fertilizer that will help your seed to resist the weeds of discouragement, setbacks and poisonous opinions. Though great effort is required, we must remember the caution from the Good Book; “work without faith is dead.”

Faith is also the promise of the universe that states that “as a man sow, so shall he reap.” It is through this promise that in faith we are able to endure the pain of effort to reap the rewards of the opportunities spring offer.

The truth is that each day our life is like a new season of spring given to us. Constantly reliving yesterday’s failures or focusing only on the today’s challenges robs us of tomorrow’s opportunities. My challenge to you is to seize each new day of spring armed with seeds of positive thoughts, and an optimistic outlook coupled with determined effort to create the life of your dreams.
